2022 W2 Shipping Address Change Request

This form is used when you are needing to change the address that will be used to ship W-2s and ACA 1095s. Once the form is completed, please save as a PDF file and email in to ClientServices@CertiPay.com.

**Please note- this is only used if you need to ship your W2s to a another address that is different from what is used for your regular check shipments. Also please be aware that our carriers will not ship to PO Boxes.**
